Timing — how long to wait after transplant surgery
Minimum: 12 months post-surgery. This gives the grafts time to complete the shed-and-regrow phase, and lets the donor scar mature to its final appearance.
Typical: 12–18 months is the sweet spot for most patients. Full graft density has stabilized, scar is mature, and pigment placement can be planned against the final visible outcome.
Longer waits (18-24+ months): Some surgeons recommend waiting longer if the transplant was particularly extensive, if healing was slower than average, or if a second transplant might still be considered. We coordinate timing with your surgeon at consultation.
Ongoing touch-ups: Once SMP is placed, plan for a single touch-up session every 5-8 years as pigment slowly fades. Not a full re-do — just a refresh.
What SMP cannot fix after a transplant
Honesty matters more than sales. SMP is not a fix for every transplant problem:
- ✗Wrong hairline placement: if grafts were placed at the wrong hairline shape or depth, SMP can only work with what’s there. Surgical revision may be needed.
- ✗Damaged follicles: if transplanted follicles were damaged during surgery and didn’t survive, SMP fills the visual gap but doesn’t bring the hair back.
- ✗Ongoing native hair loss: if your remaining native hair is still receding, SMP addresses the visible now — but you may need additional sessions as your loss progresses.
- ✗Grossly under-transplanted areas: if there are simply no grafts in a large area, SMP alone will read as a dark patch of scalp — not a natural transplant result. Additional grafts may be needed first.

Can I get SMP after a hair transplant?+
Yes — SMP after a hair transplant is one of the most common and highest-value use cases for scalp micropigmentation. Wait at least 12 months post-surgery so grafts have fully healed and the donor scar has matured. SMP fills the visual gap between the transplanted hairs with microscopic pigment dots, making the scalp appear more densely covered without the visible skin-through-hair look that suggests thinning. Most patients see 30-50% apparent density improvement.
Does SMP camouflage FUT (strip) scars?+
Yes — linear FUT strip scars respond very well to SMP. The scar tissue itself doesn't grow hair, so it visually reads as a horizontal line of bare skin across the back of the head. SMP places pigment dots inside and around the scar so the whole area matches the surrounding density. Once healed, the scar is essentially undetectable at normal viewing distance — even with a fresh razor shave.
Can SMP camouflage FUE punch scars?+

Does SMP damage transplanted hair?+
No — SMP is placed in the upper dermis (0.5-1.5 mm depth) at the follicular level, not deep enough to affect the graft structure. Thousands of post-transplant SMP procedures have been performed with no documented cases of pigment placement damaging transplanted follicles. That said — SMP should be performed by a specialist familiar with post-transplant anatomy (grafts sit slightly differently than native follicles). Not every SMP artist has extensive post-transplant experience.
