The density math

Native hair density: 60–90+ follicular units per square centimeter (varies by ethnicity and genetics).

Typical hair transplant density: 25–40 grafts per square centimeter — often less in the crown or if the surgeon opted for a conservative first pass.

The gap: even a perfect transplant delivers ~50% of native density. In good lighting or when wet, the scalp shows through the transplanted hairs — which reads as “thin,” even though it’s actually a successful transplant.

Density enhancement SMP: places pigment dots in the visible gaps between transplanted hairs. The scalp reads as densely covered because pigment fills the space that would otherwise show as bare skin.

When density enhancement is the right move

  • Your transplant is 12+ months healed and you can still see scalp between the hairs

  • Less than 12 months post-transplant (grafts still settling)

What is hair transplant density enhancement?+

Density enhancement is a scalp micropigmentation technique that fills the visual space between transplanted hair follicles with microscopic pigment dots. Even successful transplants deliver ~25-40 grafts per square centimeter (native density is 60-90+), which is why some transplants read as 'thin' or 'you can see the scalp between the hairs.' Density enhancement makes the scalp appear more densely covered without adding actual hair.

Why does my hair transplant look thin?+

Three main reasons: (1) surgical density is typically 25-40 grafts/cm² — half of native density; (2) some grafts don't survive healing (10-30% graft loss is normal); (3) the contrast between hair-bearing scalp and visible skin between hairs reads as 'thin' even when it isn't. Density enhancement SMP addresses reason 1 and 3 without requiring another surgery.

How much density improvement does SMP add to a transplant?+

Most patients see 30-50% apparent density improvement after SMP overlay. The scalp reads as fuller because the pigment dots fill the visual gaps between hairs. Under lights or in wet conditions where the scalp normally shows through, the improvement is even more dramatic.

Is density enhancement cheaper than a second hair transplant?+

Significantly. A second transplant to add density typically costs $8,000–$15,000+ and requires another surgical recovery. Density enhancement SMP costs $1,500–$3,500 and requires zero downtime. Many patients report equivalent visual improvement from SMP density enhancement vs. a second transplant at 1/4 to 1/5 the cost.

How long after my transplant can I get density enhancement SMP?+

Minimum 12 months post-transplant. This ensures grafts have completed the shed-and-regrow phase and reached their final density. Density enhancement before month 12 risks placing pigment around hairs that will shift into different final positions. At consultation we assess your specific healing timeline.

Does density enhancement work for FUE and FUT transplants?+

Yes — it works equally well for FUE and FUT transplants. The technique is the same regardless of harvest method. If you had FUT, SMP typically also addresses the linear donor scar; if you had FUE, SMP can also camouflage the dotted donor pattern in the back and sides.

Who is a good candidate for density enhancement?+

Anyone who: (a) has a completed hair transplant (12+ months post-op); (b) is unhappy with the visible density; (c) doesn't want another surgery; (d) is comfortable with the transplant hairstyle they have (density enhancement works within the existing hair length range); (e) wants a cost-effective visual improvement. Not ideal if grafts were grossly misplaced (surgical revision may be needed first).

Can density enhancement be done on just the crown, or on the whole scalp?+

Both. Many patients get density enhancement targeted specifically to the crown/vertex where transplants often leave less density. Others get full-scalp enhancement across the entire transplanted area. At consultation we design the plan based on your specific density gaps.
