SMP Explained

What Is SMP?

SMP stands for Scalp Micropigmentation — a cosmetic tattoo procedure that recreates the look of a freshly razor-shaved scalp with a natural 5 o'clock shadow using thousands of microscopic pigment dots. No surgery. No medication. No downtime.

The one-line definition

SMP (Scalp Micropigmentation) is a specialized cosmetic tattoo procedure in which thousands of microscopic black pigment dots are placed into the top layer of the scalp to replicate the appearance of freshly shaved hair follicles. The finished result looks like a natural, closely-shaved scalp with a soft 5 o’clock shadow — permanent enough to last years, temporary enough to fade naturally over 4–8 years.

SMP is NOT…

  • Not hair. SMP does not grow hair. It doesn’t add follicles. It doesn’t plug in hair strands. It’s a visual illusion of shaved follicles created entirely by pigment.
  • Not paint. The scalp is not painted black. Pigment is placed dot-by-dot, following the density and pattern of real hair — so up close it reads as texture, not a coat of paint.
  • Not a buzz cut. SMP simulates the look of a buzz cut but without actual hair length. Under a light, you see skin (with pigment on it) — not hair shafts standing above the surface.
  • Not a regular tattoo. SMP uses smaller needles, different ink chemistry, and shallower placement depth than body tattooing. A tattoo shop that doesn’t specialize in SMP will almost always produce dots that are too big, too dark, and turn blue over time.
  • Not surgery. No cutting, no stitches, no anesthesia. Topical numbing only. You walk in and out the same day, and there’s essentially zero downtime.
  • Not a hair transplant. Hair transplants surgically move real hair follicles from the back/sides to bald areas. SMP places pigment. They’re often combined — SMP camouflages transplant scars and adds density between grafted hairs.

What SMP is good for

How SMP works — the procedure, step-by-step

  1. 1. Consultation (free). We map your existing hair loss pattern, discuss the look you want, and design a hairline shape appropriate to your face and age. No pressure to book — you walk out with an exact quote.
  2. 2. Session 1 — foundation. Your scalp is shaved to a completely smooth finish before pigment work begins. Joe places the first pass of microscopic pigment dots — hairline design, mid-scalp density, crown blend. Session runs 2–4 hours depending on coverage area.
  3. 3. Heal (1–2 weeks). Pigment settles as your skin heals. Some dots fade slightly and settle to their final shade. You look presentable within 24 hours; the healed final look emerges over 7–10 days.
  4. 4. Session 2 — refinement. We add density on top of the healed foundation, tighten the hairline, and correct any spots where pigment didn’t fully take. The result looks noticeably fuller.
  5. 5. Session 3 — finishing (if needed). Final pass to perfect the blend, soften the transition into the remaining side/back hair, and lock in the completed look. Not every client needs a third session — simpler cases finish at two.
  6. 6. Touch-up (5+ years later). As pigment slowly fades, most clients return for a single-session refresh every 4–8 years. Never a full re-do — just topping up.

Where SMP came from

Scalp micropigmentation as a distinct discipline emerged in the early 2000s — pioneered by the UK-based HIS Hair Clinic and refined in the mid-2000s to early 2010s by artists in North America and Europe. Modern SMP uses machines, needles, pigments, and technique protocols specifically engineered for the scalp — which is why the results today look nothing like the crude “dot tattoos” from the early era.

SMPCA opened in 2016 as one of the Bay Area’s earliest dedicated SMP studios. In the years since, Joe V. Nguyen has performed hundreds of procedures across hairline restoration, Norwood coverage, alopecia work, women’s density, and hair transplant scar camouflage — earning AAM Gold Certification, BBB Accreditation, and County of Alameda Public Health licensure along the way. Today SMPCA operates two studios: the original East Bay location in San Lorenzo and a downtown Palo Alto studio serving the Peninsula and Silicon Valley.

SMP — Frequently Asked Questions

What does SMP stand for?+

SMP stands for Scalp Micropigmentation — a cosmetic tattoo procedure that places thousands of microscopic pigment dots into the scalp to recreate the appearance of freshly shaved hair follicles.

What is SMP, in one sentence?+

SMP is a specialized cosmetic tattoo that gives the look of a freshly razor-shaved scalp with a natural 5 o'clock shadow — used to cover baldness, thinning, alopecia, or hair transplant scars without surgery, medication, or downtime.

Is SMP the same as a regular tattoo?+

No. SMP uses a specialized machine, needle sizes, pigment formulations, and depth of placement designed specifically for the scalp — not the same as a body tattoo. A regular tattoo artist without SMP training will almost always produce a bad SMP result (dots too large, ink placed too deep, wrong pigment shade).

Does SMP grow hair?+

No. SMP does not grow hair — it visually replicates the look of shaved hair follicles using pigment dots. If you want actual hair growth, you're looking at hair transplant surgery or medications like minoxidil / finasteride. SMP is often combined with those approaches to add density and camouflage scars.

How long does SMP last?+

SMP typically lasts 4–8 years before fading enough to warrant a touch-up. Pigment slowly breaks down as skin cells turn over. Most clients come back for a single-session refresh every 5 years.

Is SMP painful?+

Most clients describe it as a mild scratching sensation, less intense than a traditional tattoo. We use topical numbing throughout the session. Sessions typically run 2–4 hours depending on coverage area.

How many sessions does SMP take?+

Most SMP treatments take 2–3 sessions spaced 1–2 weeks apart. Density is built gradually so the result heals naturally rather than looking painted on. Complex cases (alopecia totalis, extensive scar work) may need 4 sessions.

How much does SMP cost?+

SMP pricing varies by coverage area and number of sessions. A hairline touch-up starts around $800; full Norwood 6-7 coverage runs $2,500–$4,000; scar camouflage typically $600–$1,500. Every SMPCA consultation is free and includes an exact quote.

Who is SMP for?+

SMP works for anyone with hair loss or scalp visibility they'd like to camouflage — men with pattern baldness, women with diffuse thinning, alopecia patients, hair transplant patients wanting scar coverage, and anyone with a scar on the scalp from surgery or injury.
